本故事发生在清朝末年，靖王爷（徐连升 饰）为救爱女（朱亚娜 饰）性命，不得不病急乱投医，找来了沧州的乡下郎中喜来乐（李保田 饰）。没想到被宫里太医王天和（杜雨露 饰）断为绝症的格格，喜来乐竟用奇招救活。喜来乐本是与世无争，根本不想留在京城这个是非之地，但天意弄人，靖王爷不但用计把他留下，更是把他全家老小和沧州的“一笑堂”搬来京城。从此一向目中无人的王太医就与喜来乐结下了梁子，屡屡对喜来乐暗下毒手。命大的喜来乐不仅每每躲过暗算，更是凭借自创的偏方怪招在京城获得了神医的名号。靖王爷为了让喜来乐安心留在京城，更是把他在沧州的老相好寡妇赛西施（沈傲君 饰）和她的店“食为天”搬到了“一笑堂”的对面。喜来乐爱吃醋的老婆胡氏（梁丽 饰）本就是个不好惹的母老虎，赛西施的到来，更是惹得他们全家每天鸡犬不宁。喜来乐的名气越来越大，甚至得到了皇帝和慈禧的赏识，王太医也加紧了陷害他的脚步，前方等待喜来乐一家人的究竟是什么呢
